Liability Waiver

I understand that participation in the Amazeum program involves potential, although highly unlikely, loss or damage to personal property and bodily injury. In consideration of myself or my child being permitted to participate in the scheduled activity, I hereby release and hold harmless the Amazeum as sponsor and activities provider and its Released Parties from any and all actions, damages, claims, or demands which I or my child(ren), our heirs, executors, administrators or assigns may have against the Released Parties for all bodily injuries, known misconduct or grossly negligent act of any of the Released Parties. In case of emergency, I hereby authorize the Amazeum, its Released Parties, paramedics and/or the nearest hospital emergency room to administer medical attention to the below named minor. I further understand that the Amazeum or any of its Released Parties are not responsible or liable for injuries sustained on the premises. I, the undersigned, have read this release and understand its terms. I realize this is a partial release of liability, and limits, to some extent, my own rights to sue in the event of any loss or injury, and I execute it voluntarily and with the full knowledge of its significance. I agree to follow, or cause to be followed, all directions of the Amazeum program leaders.

Photo Release

I hereby freely and voluntarily consent to the use, publication and public display of my or my children’s name and/or the image of me or my minor children’s likeness by the Scott Family Amazeum, and its commissioners, officers, trustees, employees, affiliates, sponsors and agents (the “Released Parties”) for any and all purposes, including, but not limited to promotional, advertising, education and trade purposes, through any medium or format, and for any purpose whatsoever. I give this authorization and release without expectation of compensation or based on any other inducement, promise or representation made by the Amazeum or its Released Parties. I waive any right to see or approve any recording of my or my children’s likeness and any materials in which my or my children’s likeness is used. I agree and understand that my or my children’s likeness may be modified and that my or my children’s own name, a fictitious name, or no name may be used in association therewith. I irrevocably release, discharge, and hold harmless the Amazeum, and its Released Parties from any claims, demands, or causes of action that I or my minor children may now have or may hereafter have for defamation, slander, libel, invasion of privacy or right of publicity, copyright infringement, or any other right arising out of or relating to the use of my or my children’s likeness. I understand and agree that upon execution of this release, the image(s) shall become the sole property of the Amazeum, to use for any purpose in its sole discretion.
